Working in conjunction with GP surgeries, this service provides non-medical support and advice to older people and their carers.

About this service

Our team of Social Prescribers work closely with GP surgeries in the Lewes, Havens, High Weald and Greater Wealden areas, with the aim to improve the well-being of those aged 50 and over, and carers of any age. We help people find non-medical support to improve their health and wellbeing. Instead of giving medicine, we help connect people to local services, activities and offer practical help and assistance.

Our team is supportive, attentive, and highly skilled at listening. We are approachable, knowledgeable and highly trained, providing professional and person-centred care. We offer guidance, information, and advice on a wide range of topics, including community activities, social care, and wellbeing services.

Our goal is to make life a bit easier, reduce stress or loneliness, and help you feel more supported in your community.

What we do

❖ Listen and Assess Needs: We spend time with individuals to understand what matters most to them, beyond clinical symptoms, and identify the support that will make the biggest difference.

❖ Connect to Local Services: We link people to community groups, clubs, classes, volunteering, charities and social activities to meet new people.

❖ Support Life Challenges: We provide guidance and support to access services on loneliness, housing, debts, benefits, employment, caring responsibilities, and specialist services.

❖ Wellbeing and Self-Care: We support healthy lifestyles and help people find wellbeing activities and exercises to lift mood, improve health, and obtain support for bereavement or other emotional and mental health support needs.

❖ Support Independence: We help people access services for home safety, mobility aids, home care assessments, carers, and community transport.

❖ Access Appropriate Services: We provide support and guidance to help people access housing, benefits advice, and other specialist services.

Volunteering with Us

Currently we have no volunteering opportunities within Social Prescribing - however if you are interested in volunteering with us, there are several other opportunities - please contact our HR team.

Collaboration with Healthcare Teams

We work alongside GPs, nurses, and other healthcare professionals to provide holistic, person-centred care, ensuring that social and health needs are addressed together.
