Free support during hospital discharge for people 60+ in Harrow and Brent
A safe return home after your hospital stay
Leaving hospital can be a vulnerable time. If you’re 60+ and returning to Harrow or Brent from Northwick Park Hospital, our team can provide practical support to help you settle back in safely and confidently.
Our trained support workers will accompany you home and help with simple tasks on the day, to make your return as smooth as possible.
This is a free service delivered in partnership with Harrow Carers.
What we offer
● Escort home from hospital
● Help with essential shopping
● Changing bed linen
● Support with opening and managing post
● Preparing a light snack and drink
● Follow-up phone calls for up to 3 days after discharge
We’ll make sure you feel supported, heard, and looked after during those crucial first few days at home.
