The team supports you once you have left hospital, supporting you with a variety of tasks except personal care. (Not a same day service)
Once you have been discharged from hospital, if you live in the borough of Hillingdon and are 60+, our Home from Hospital Team are there to offer a range of practical support to help you on the road to recovery for up to six weeks.
