Day 9 – Shining Stars
Our Connect team delivers a community-based prevention service that helps people aged 65+ and adults with neurodiverse conditions stay independent at home - delaying or avoiding the need for more intensive support.
This year, over 3,000 people benefitted from the service, including many supported through complex hospital discharges by our Hospital Link Worker. We also helped secure over 600 successful Blue Badge applications, giving people greater freedom and independence:
“[The worker] spent time filling in my claims for a Blue Badge Parking Permit and Attendance Allowance. Both were successful and have improved my lifestyle. He was very helpful, patient and took time to help me as I cannot hear very well.”

Case Study – L’s Story
L, in her 70s, lived with her adult daughter who has learning difficulties. After losing her husband, L faced anxiety, depression, and financial hardship. She rarely left home due to health issues.
The Connect team built trust and provided tailored support:
- Scam awareness and registration with the Telephone Preference Service
- Energy-saving measures and a boiler check via Warm & Wise
- Smoke alarms fitted by Nottinghamshire Fire and Rescue Service
- Financial help through bereavement support, Attendance Allowance, Pension Credit, and reduced council tax
- Referral for community transport
When the case closed, L and her daughter were warmer, safer, financially better off, and more confident - with a clear understanding of local support available.
