Community Support Worker
Hours: 37 hours per week
Salary: £23,876.84 per annum
Based: Home and field based (focus on Nottingham City)
Tenure: Fixed Term until 30th September 2026
About the role
Are you passionate about empowering people to live independently and improve their wellbeing?
We’re looking for a Community Support Worker to join our Wellbeing at Home service – our short-term community support service is designed to help adults maintain independence and improve health and wellbeing.
As a Support Worker, you will:
- Provide practical and emotional support to individuals to help them achieve their goals.
- Work collaboratively with service users to develop and follow short-term support plans.
- Promote independence, wellbeing, and social inclusion.
- Support individuals to access community services, benefits, housing, and healthcare.
- Maintain accurate records and contribute to service monitoring and evaluation.
About you
We’re looking for someone who is:
- Has experience in a support or care role (paid or voluntary).
- Communicates effectively and builds trust quickly.
- Is organised, reliable, and able to work independently.
- Has a good knowledge of local services and resources (or is willing to learn).
- Is able to work in a hybrid way, travelling across Nottingham City as required.
