Connect Coordinator
Hours: 37 per week
Salary: £20,520 per annum
Scale: AUNN Band D
Based: Home & field based
Tenure: Fixed term until July 2023
About the role
The Connect service provides support to people at risk of losing independence because of finances, housing, physical and mental health, social isolation, care needs or any other issues concerns affecting their wellbeing. We currently have an opportunity available within the team, which could be an excellent development opportunity for the right individual. The successful candidate will provide telephone and face to face support to empower people to find solutions to their problems.
About you
With an awareness of the issues vulnerable people face, you will be empathetic, sensitive and able to empower people to self-manage their needs. Self-motivation and discipline are key as you will work from home and manage a busy caseload. Ability to drive and access to a vehicle are also essential (for applicants who are unable to drive due to a disability, reasonable adjustments will be considered).
