We’re looking for a Senior Community Support Worker to join our Connect Service – a short-term support service designed to help adults maintain independence and improve health and wellbeing. The service prioritises older adults and those with long-term conditions, learning disabilities, autism, neurodiverse conditions, and acquired brain injuries.

As a senior member of the team, you will:

  • Act as a key point of support for our Support Workers, offering guidance, mentoring, and leading reflective practice sessions.
  • Lead on triaging new referrals, ensuring timely and appropriate responses to individuals’ needs.
  • Provide direct support to individuals via telephone and face-to-face contact, helping them to identify solutions and build confidence in managing their own wellbeing.

About you

We’re looking for someone who is:

  • Empathetic, sensitive, and confident in working with vulnerable adults.
  • Self-motivated and disciplined, with the ability to manage a busy caseload while working from home.
  • A strong communicator and team player, with leadership potential or experience.
  • Is able to work in a hybrid way, travelling across Mid and South Nottinghamshire as required.
