We offer a Memory Support Service to anyone who is concerned about their memory, but who doesn’t have a diagnosis of dementia.
To qualify for this service, you need to be referred by your doctor (or another medical professional). This is to make sure that there are no underlying medical reasons for your memory concerns.
Once you are referred, one of our friendly and experienced advisers will arrange a phone appointment during which they can provide information and support around a broad range of topics and can signpost you to other organisations if necessary.
What is the Memory Support service?

Once you are referred, one of our friendly and experienced advisers will arrange a phone appointment during which they can provide information and support around a broad range of topics and can signpost you to other organisations if necessary.
Some of the topics they may cover include:
• Advice on making healthy lifestyle choices to support brain health
• Suggestions for memory-related coping strategies
• Ideas for keeping safe and well
• Information about local groups and activities to help you keep socially connected
• An overview of the support available from health, social care & other local services
• Suggestions for assistive technology and using aids around the home
• A benefits check and how to claim benefits you may be entitled to
• Advice on planning ahead, including discussions around Power of Attorney /Advance Care Planning.
If you are waiting for an appointment with the memory clinic, our advisers are also happy to chat through what might happen at the appointment and discuss any concerns you may have.
To be eligible for this service, you must be registered with a doctor's surgery within Oxfordshire.
Please be aware that this service is provided by our dementia support service, known as Dementia Oxfordshire. This is because the Dementia Oxfordshire team has a great deal of experience dealing with memory issues and practical support strategies. It does not mean that everyone with memory problems has dementia.
