Helping People Home from Hospital: Spotlight on Our Take Home and Settle Service

This week we’re shining a light on a vital service that quietly makes a big difference every day: our Take Home and Settle service, which operates across Bromley, Greenwich and Lewisham.
When someone is discharged from hospital, the journey doesn’t end at the hospital doors. For many older people, those first few hours and days at home can feel overwhelming, especially if they live alone, have mobility issues or are recovering from illness. That’s where our Take Home and Settle team steps in.
Working closely with local hospitals including Queen Elizabeth Hospital, Lewisham Hospital and the Princess Royal University Hospital, our trained support staff help patients get home safely, make sure they’re warm and comfortable, and offer practical support that can include:

Turning on heating and lights

Making a warm drink or light meal

Picking up medication or discharge papers

Collecting property left behind

Doing a small shop for essentials

Checking back in the next day
As one patient told us:

This service is available by referral only from hospital or medical professionals. We do not accept self referrals, but we are proud to work in partnership with NHS teams to ensure that those most in need receive the right support at the right time.
Our team offers more than practical help. They provide reassurance, kindness and a human connection that helps people feel safe and supported in those crucial first days after hospital discharge.
