Support Worker

Are you passionate about supporting older & vulnerable people? Do you want to work for a local independent charity?  

Age UK Buckinghamshire are looking to recruit Support Workers to contribute to a new project supporting vulnerable older people in Buckinghamshire who have been in hospital and are now ready for discharge.

This role will be crucial in providing the practical support that some patients require to get home safely and be able to settle back in their homes following a stay in hospital.

Based at Stoke Mandeville Hospital, you will be there to support older people who have been admitted and are now ready for discharge, or those who find themselves in A&E. 

As part of this service, you will be required to provide a range of services relevant to the individual patients needs including:

  1. To facilitate hospital discharge by transporting patient’s home
  2. Complete settle in visits
  3. Complete follow up calls & community support
  4. Complete assessments for further care needs.

Requirements:

  • Experience in the similar roles (ideal)
  • Authorised to work in the UK
  • Must hold UK driving Licence
  • Must have access to a vehicle to transport patients’ home

Please note: There is no personal or clinical care involved in this role.

Shifts & Schedule:

Location: Stoke Mandeville Hospitals plus service user homes.

Shift Times: Various shifts available. Hours of operation are 11 a.m. – 7 p.m. Monday – Sunday. Some weekend work is expected.

Both Full & Part time permanent contract positions available. Casual flexible contract hours available too.

Pay Rates: £ 9.90 p/hour plus 5% pension contribution
