Information and help for people living with dementia and their carers.

About the Service

The Kirklees Dementia Hub is a partnership between Community Links and Age UK Calderdale & Kirklees and offers information, advice and support for people who are living with dementia and their carers, family and friends.

We can provide appointments in the following ways:

🧍Face to Face appointment in the community
🏠 Home Visits (A worker will agree this depending on needs)

Please be aware we cannot guarantee an appointment on the same day, please contact us on the number below prior to attending where we can advise on available appointments on the day. If we are unable to provide you with the appointment on the day we will arrange an appropriate alternative.

Aims of the Service

The aim is to ensure people within the Kirklees community have access to a wide range of dementia information and advice.
Our partners Age UK Calderdale and Kirklees run Dementia Friends sessions within the local community.

Examples of what we do:

Dementia Friendly Community Steering Group:
We are working towards creating a Dementia Friendly Kirklees, this will support those living with dementia to remain living well in Kirklees and to continue accessing their local communities. The Kirklees Dementia Friendly Community Steering Group is open to all in Kirklees to join and support each other in creating change and improvements for those living with dementia in Kirklees. We can improve accessibility, raise awareness and support those with dementia and their carers to be heard and understood!

DEEP:

A monthly meeting for those diagnosed with dementia to meet others on their dementia journey, enjoy activities and being together as well as a safe space to share your views and take part in opportunities to create change in Kirklees and be heard on what you want to see in your local area!

Volunteer With Us!:

We have lots of volunteering opportunities! To be involved in supporting those living with dementia or share you own lived experience with others starting their journey, please contact 01484 411 074 for more information!

Dementia Awareness Raising:

The Kirklees Dementia Hub is working with local businesses and groups to make Kirklees more dementia friendly. They offer free Dementia Friends sessions within the local community. Call to book your free session!

Referrals:

Referrals into the service are made through the SWYPFT Memory Service, a GP/health professional or you can also self-refer.

Referrals can be made for adults of any age, who have been recently diagnosed with dementia.

Opening Times:

The Service is open from Monday to Thursday 9am-5pm and Friday 9am-4.30pm, with the exception of recognised public holidays and will be supported by an office hours telephone advice line.
