We work with people who are living with complex health and social needs to identify services that can help you maintain your health and promote wellbeing. We are a team of Community Nurses and Age UK Health & Wellbeing navigators.

We can work with you to:

•      Reduce the risk of falls

•      Access support with everyday tasks  

•      Reduce the risk of hospital admission  

•      Feel in control of your life

•      Reduce the risk of social isolation

•      Connect to others in the community

•      Manage your own health needs

What we will do

An assessment will be undertaken which includes a health, mobility and medications review, memory screening and a discussion about wishes and preferences for your future care. The agreed plan is then shared with the GP and other relevant professionals with a view to provide ongoing support. 

Avoid hospital

You may have been told that you have the condition healthcare professionals refer to as frailty. As people age they can become weaker and more vulnerable. Small events such as an infection or a fall can lead to a dramatic change in health resulting in a hospital admission and a lack of independence.

How we help

The assessment will help identify physical and mental health services as well as social care interventions to prevent hospital admission and help maintain your independence.  This could include encouraging a healthy lifestyle; supporting engagement with social, leisure and recreational activities; providing support with managing social needs, finances, housing and connecting with others.
