Helping Older Residents in Harrow & Brent Return Home Safely

Age UK HHBs Hospital Discharge service, a lifeline for older patients in Harrow and Brent has recently been featured in Harrow Online News. The initiative not only assists individuals as they transition from hospital to home but also ensures valuable hospital bed space is freed for those in need.

In collaboration with Harrow Carers and the local hospital trust, the service supports residents 60+ following their discharge from Northwick Park Hospital. The service is designed for patients who are medically fit to leave hospital but require extra help to settle back at home.

A dedicated team of nine works tirelessly to support these patients, helping them with the journey home and making sure they are comfortable. Their assistance ranges from providing essential shopping to helping out around the house, making the return as smooth as possible.

Pam Smith, a member of the team, shared: “We’re often the first service through the door when a patient goes home.” She explained that a team member accompanies patients home by taxi or Hospital Transport, ensuring there is food in the fridge and that they are settled in. If additional support is needed, Age UK HHB can connect individuals with further services, including internal Age UK HHB services, Harrow Carers Services, or other community resources.

The service doesn’t end at the doorstep. Patients receive three follow-up calls post-discharge to monitor their wellbeing, and if further assistance is required, a home visit can be arranged to explore additional support options.

Each month, the team supports over 80 patients, with funding provided by NHS North West London. The service operates Monday to Friday, from 10:00 to 17:30, with referrals accepted until 16:00, ensuring continuous care for Harrow’s older residents.

This vital service exemplifies Age UK HHB’s commitment to supporting older people in the community, offering reassurance and practical help when it matters most.
