GM Health & Care Champion Awards 2025

We are delighted to announce that Sue, our Services Development Manager, has been nominated for the Greater Manchester Health and Care Champion Awards 2025, under the innovation category. The nomination is for her hard work co-ordinating our Home from Hospital service.

Home from Hospital is our hospital discharge support service. It supports socially isolated older patients after discharge at Manchester Royal Infirmary (MRI), Wythenshawe Hospital and, increasingly, North Manchester General (NMGH).

Not only does the service support with things like emergency food parcels, donations of important white goods (fridges etc), fitting of key safes for carer access, it also means that patients can return home after discharge rather than staying in hospital for longer than needed.

Without the support of the service, many patients would end up overstaying in hospital, due to having no support network and low resources.

The support doesn’t stop there. Our Home from Hospital team signpost patients to relevant services to reduce the likelihood of them being re-admitted to hospital, for example, our Advice service, Ageing Well, and care services. The service supports, on average, 20-25 people a week, meaning that we help around 1000 vulnerable patients to get home quicker, and getting them the support and care that they need. And also, it frees up much-needed hospital beds.

The GM Health and Care Champion Awards recognises “…outstanding contributions from our whole health and care workforce…” and celebrates the individuals and teams who work on the frontlines and behind-the-scenes to improve the health and wellbeing of others.

We are delighted that Sue and the team’s hard work has been recognised and are thankful that the support given to some of Manchester’s most vulnerable people has been identified and celebrated.

The Awards Ceremony takes place on the 10th July. It will be a brilliant evening that celebrates and recognises the work of Health and Care staff in Greater Manchester, who do so much to support the wellbeing of the city’s residents.
