In the role of Reminder Finders Navigator you will:

Support Reminder Finders Facilitators with any enquiries about groups, and make initial appointments with participants for assessments
As a team, facilitate of Reminder Finder sessions on a weekly basis, ensuring participants are engaged during the workshop, encouraging interaction and peer support but providing one to one support if individuals are struggling
Prepare material needed for each session
Engage with Carers before and after sessions, ensuring that they have adequate support and information about other groups and opportunities within Age UK Wirral
To quickly identify and support carers through crisis
Support the Reminder Finders Facilitator with assessing and evaluating the impact of the workshops on both carers and participants.
Keep accurate records of telephone calls, visit and referrals, to compile a monthly statistics report.
Maintain strong links with community and voluntary groups and statutory organisations so that up-to-date signposting information can be provided to carers who want to engage with the wider community.
Manage infection prevention control issues through adhering to PPE requirements
To identify and celebrate the impact of our work and present this evidence as case studies and examples of good practice.
