£16,487 per annum paid monthly in arrears per annum, paid monthly in arrears
21 hours per week, actual working pattern to be agreed
Fixed term contract to 27th February 2026
Our falls prevention pilot project aims to reduce the risk of injurious falls and the long-term impact of falling in older people. The pilot project is being delivered in the west area of Blackburn where there are larger numbers of older people, at risk of falling.
We are looking for a Falls Link Worker for the delivery of the project, which aims to make a difference to those who are at risk of falls in the community. The main purpose will be supporting individuals to remain independent in their homes and empowering them to take charge of their health, wellbeing and independence, offering and co-ordinating a range of support, including carrying out home visits and completing holistic assessments. The Link worker will also support the delivery of a community engagement Falls Awareness campaign to increase referrals into the project and raise awareness of falls risks among older people.
The successful candidate will have experience in completing assessments and working in partnership with health or social care professionals and is able to motivate older people to engage in new activities and services. An understanding and interest in falls prevention for older people would also be advantageous.
