Going Home Link Worker

17.5 hours per week

Flexible over Monday to Friday.

Salary FTE: £25,806.56 per annum, pro rata.

Join our friendly ‘Going Home’ team to support people to return home from hospital.  This exciting new role will be based in a Bristol hospital.  We are looking for someone who enjoys being part of a dynamic team and who has great interpersonal skills.  Ideally, you’ll already have in-depth knowledge of the services offered by the voluntary sector in the city, but if not, you will be supported to learn this too.  You will be working with other organisations in the health, social care and voluntary sectors to enable people to return home safely and on time with the support they need to live independently. This role is all about team work, for the benefit of individuals, in a fast paced environment. If you like working directly with people, are a good listener and effective communicator, and know how to build strong relationships with other organisations then we’d love to hear from you. 

We work closely with other voluntary organisations who share our ambition to build an age-friendly city.  We are looking for someone with the drive and commitment to help us make a difference. 

As Hospital Link Worker you will ensure that people’s needs on return home are known and supported ahead of them leaving hospital.  You’ll support health staff to develop a welcoming environment for people, support them by listening to their needs, make referrals to appropriate services and connecting them to the opportunities and support available in their communities.  Due to the nature of this post, you will be required to work mainly from either the BRI or Southmead Hospital, but also to come to the Age UK Bristol office to ensure close connections with your team.  There will be some opportunity for working from home.

You will be working with a friendly cross-sector team of professionals within the hospital and supported by a friendly team of people who are based at Age UK Bristol’s office in Victoria Street, Bristol and The Withywood Centre, Queens Road, Withywood.
