Service Coordinator

Are you passionate about supporting older & vulnerable people? Do you want to work for a local independent charity? Age UK Buckinghamshire are looking to recruit a Service Coordinator to contribute to a new project supporting vulnerable older people in Buckinghamshire who have been in hospital and are now ready for discharge. The service delivers transport home; settle in visits; follow up and community support.


The Service Coordinator will act as the point of contact for the ward teams and A&E for referrals. The coordinator will then coordinate the support workers and volunteers to deliver the service – and will be part of the delivery themselves.

A key part of the role will be to ensure there is good communication with patients and clinicians. You will also need to monitor delivery; oversee record keeping and ensure service quality.

Requirements:

  • Experience in the similar role.
  • Authorised to work in the UK
  • Must hold UK driving Licence

. Must have access to a vehicle to transport patients’ home

Please note: There is no personal or clinical care involved in this role.

 

Shifts & Schedule:

Location: This role will cover both Wexham Park & Stoke Mandeville Hopsitals and service user homes. The role may also require working from the head office in Aylesbury.

Contract: Permanent Contract 28 – 35 hours per week. Days and times to be agreed, but some evening and weekend work to be expected.

Pay Rates: £ 12.00 p/hour plus 5% pension contribution
