Joined up Care Derbyshire – Dementia Engagement

Joined Up Care Derbyshire, also known as Derbyshire’s Sustainability and Transformation Partnership (STP) brings together Health and Social Care organisations across Derbyshire. They are engaging on the priorities for our Derbyshire Well Pathway for Dementia. They want to find out what is important to people living with dementia, their carers and those staff who support them.
