Job Description

Team purpose:            To work collaboratively across services and projects to ensure consistent and holistic assessments and reviews are carried out with people accessing Age UK Doncaster’s activities using a strengths-based approach. To continually develop our approach in line with Local and National legislation, guidelines and practices. 

Job purpose:   The Assessment Officer will work across the organisation to carry out assessment and reviews with people accessing our services and projects. You will work closely with the Senior Assessment Officer to continually develop the approach we use in line with the agreed policies and procedures of Age UK Doncaster and in a manner which promotes the dignity and independence of the people we support.    

Main duties: 

  • Carry out initial strengths-based assessment for people accessing our services and projects. 
  • Formulate appropriate and proportionate support plans 
  • Signpost individuals internally and to external support where appropriate 
  • Accurately record information and keep all systems up to date   
  • Liaise with the people we support, their important individuals and next of kin as required 
  • Liaise with statutory agencies as required and maintain professional relationships 
  • Support the Senior Assessment Officer to evolve, develop and embed new ways of working where required 
  • Work closely with the wider team to ensure appropriate cover at all times for all services as per the Continuity Plan.   
  • To undertake any other reasonable duties in line with the job purpose that may from time to time be required by management.  

Hours of work: 21-35 hours per week Monday to Friday, to be agreed 

Rate of pay: £22,000 Per Annum
