We're commissioned by North Central London Clinical Commissioning Group to support people newly diagnosed with dementia, so work very closely with Enfield Memory Service.

Health and Wellbeing Navigators support people newly diagnosed with dementia. We also support people who are concerned they have memory problems,or who are seeking diagnosis, and their families.

Being diagnosed with dementia can be a challenging time, and it can be hard to know where to start finding support for the person diagnosed, and their family / carers. It can be useful to reduce stress and anxiety to have someone who understands the condition and knows the services that are available to help you to live well with dementia.

Anyone can refer to us, but if you have been seen at Enfield Memory Service they will immediately refer with your consent. You can refer yourself or a family member and it can be some time after a diagnosis, as we are aware sometimes you don't need any help at that point, and questions arise later.

When someone is referred to the service, one of our navigators will meet with the individual and the

Understanding your diagnosis
Living with dementia
Understanding the progression of dementia
Caring for someone with dementia
Welfare benefit checks and entitlements including Council Tax exemption
Housing options
Later life planning
Accessing care services
Keeping well and active
Connecting with others
We will also be able to answer any specific questions or queries you might have. From the basis of this conversation, we will develop a plan of how we can best support you and put you in touch with other organisations who can also help. We will be able to work with you for up to 12 weeks (about 3 months). Age UK Enfield also run a range of services that are suitable for people with dementia and their carers, that we can link you to like Memory Meet Up, held fortnightly in Enfield. We will always look for other dementia friendly activities in the borough that are suitable and close to where you live.

Whilst we offer support to people newly diagnosed with dementia, we also welcome enquiries from people at any time, it could be a while after diagnosis, or if the home situation or behaviours have changed. We are a team of trained professionals, that understand how dementia affects families and work with you to help you live well.

We also provide dementia awareness sessions in the community, so people are aware of the signs, symptoms, behaviours and know when to seek help. There is no charge for this service
