Our service is not entirely located at Age UK Bristol's offices, as Link Worker staff work on an outreach basis from two local hospitals (Bristol Royal Infirmary and Southmead Hospital) as well as in Bristol City Council's Adult Social Care 'front door' service in City Hall. The team is comprised of a Manager, Link Workers and an Admin Support Officer. In the hospitals, Link Workers receive referrals from NHS case managers, therapists and the acute discharge teams. They support people to return home by providing advice, signposting, and access to a wide range of practical and social support. In Adult Social Care's front door, our Link Workers take referrals from the council's call handlers and social care practitioners to connect people to help from the voluntary and community sector. Our Link Workers know the voluntary and community sector well and can ensure that people get the support they need and often prevent the need for more costly and unnecessary interventions from health and social care colleagues. Age UK Bristol is funded by Bristol City Council from the Better Care Fund to provide this service.
