Take Home and Settle Assistant  Lewisham - Shift 4

Two week rota

Week 1: Monday & Tuesday 17:00 - 21:30

Week 2: Mon, Tue, Fri 17:00 - 21:30 and Saturday & Sunday 09:00-17:00

 Salary:

Annual retainer fee of £3,502. You will be paid in addition for call outs at £13.15 per hour Mon-Fri and £15.18 per hour Sat/Sun

Are you compassionate, self motivated and practical with a desire to help older people?

We have an opportunity available to work for the Take Home & Settle Lewisham team as an assistant facilitating clients safe discharge from hospital to home.

You will support clients by accompanying them with any small items of equipment back from the University Lewisham Hospital and other local hospital sites. You will provide practical and emotional support and ensure that they are left in a safe warm environment with access to adequate food provisions.

This post is home based and requires you to have a driving licence and your own car as well as a knowledge of the Lewisham borough.
