Hospital Discharge Support Volunteer

Help make a difference to the older people of Buckinghamshire and join our Hospital Discharge Service as a volunteer!
As a volunteer you will be joining our teams freeing up beds by discharging patients safely as well as providing ongoing support to foster independence, improve wellbeing and minimise the risks of early readmission.

The service is funded by Buckinghamshire Council and is in joint partnership with Age UK Buckinghamshire and Age UK Hillingdon Harrow & Brent.

This service supports patients being discharged from both Stoke Mandeville and Wexham Park Hospitals. Our friendly Hospital Discharge Support Team will transport people home and help them settle in after their hospital stay. Our Community Support Team can provide 4-6 weeks of support at home to help with shopping, light housework and emotional support in order to prevent readmission and improve independence at home.
