Admission Avoidance Support Worker Required Covering South Cambs., Cambridge City & East Cambs.
Fixed term for 1 year with potential to extend.
Age UK Cambridgeshire and Peterborough is an independent charitable organisation which exists to improve the quality of life for older people across the county.
Working in partnership with the Voluntary Sector Alliance, the Hospital Discharge & Admission Avoidance Support service is focused on supporting hospital discharges and patient recovery at home, additionally offering community based holistic and flexible support for older people in vulnerable situations to help prevent hospital admission, re-admission or use of emergency services.
We are looking for someone to work within the VSA to support an older person who has been referred from the Urgent Emergency Care Hub. The successful applicants will be supporting older people 1:1 within their homes with welfare calls and home visits, responding to urgent and essential needs with the aim of supporting an admissions avoidance.
About
- 35 hours per week Monday to Friday (part-time hours would be considered).
- Hybrid role with travel predominately in South Cambs., but extending throughout the whole county as required, two days per week based at our head office in Chatteris.
- £25,207 per annum + benefits.
- The successful candidates will be required to undertake a Disclosure and Barring Service check at an Enhanced level.
