The Hospital & Community Navigation Service may be able to help your clients by:
- Support safe discharge of patients from hospital to home
- Provide wellbeing checks and home visits
- Help to arrange home adaptations
- Support them in accessing other services
- Collect and deliver prescription medication
- Help arrange safety equipment; such as pendant alarms and fire safety checks
