Personal Independence Coordinator
The Integrated Care Programme and this role is a key enabler of personalised planning and offers encouragement and support for individuals to self-manage and self-care using the best practice available and to offer a continual supportive presence over a period of up to 12 weeks.
The successful applicant will form a key role within a multi -disciplinary team, providing regular contact, liaison, brokerage and support planning to identify patients, their carer’s and family.

The service is based in local centre’s where older adults attend, providing opportunities for older adults to be involved in stimulating activities that enable them to live independently at home by learning or maintaining daily living skills and to remain a part of their local community for as long as possible.

Age UK Lancashire delivers a Veterans & Families project in Blackpool, Fylde & Wyre “Removing Barriers” funded by the Armed Forces Covenant Fund. This service supports families and carers of older veterans where we will be on hand for up to 12 weeks.
