    The successful candidate will work across the community to support patients across Central Lancashire, by taking them home following their discharge from hospital, ensuring they are safely settled back in their homes and to provide extended support where required.

    The service is based in local centre’s where older adults attend, providing opportunities for older adults to be involved in stimulating activities that enable them to live independently at home by learning or maintaining daily living skills and to remain a part of their local community for as long as possible.

  • Dementia Activity Leader

    Age UK Lancashire  has been successful in a bid to provide Cognitive Stimulation Therapy (CST) sessions, working in partnership with the local CCG & national Age UK charity. Sessions are clinically proven to improve the quality of life for people living with mild to moderate dementia.

    We are delighted to be able to recruit a team of Dementia Activity Leaders. You will be responsible for running sessions, planning session content, administration, client assessments and monitoring to ensure suitability. Training for this role will be provided.
