About the Role
We are seeking a compassionate, organised, and proactive individual to join our team as a Hospital Discharge Grant Carers Support Worker. This is such a rewarding role, and you will play a key role in helping carers apply for Hospital Discharge Grants to support them during the transition from hospital care back to home life, working closely with local hospitals, Leicester City Council, health and social care professionals, and community organisations.
Key Responsibilities
You will develop, coordinating, and deliver a support scheme for adult carers (aged 18+) in Leicester whose loved ones will be in hospital or recently discharged.
You will play a Develop and coordinate the Hospital Discharge Grant scheme within Leicester.
- Provide 1:1 support to carers over the age of 18 to help them understand and apply for grant funding.
- Ensure timely and accurate completion of grant applications.
- Maintain accurate records and monitor outcomes in line with data protection and safeguarding guidelines.
- Raise awareness of the scheme through outreach and partnership working.
- Work flexibly to meet the needs of carers, which may involve occasional visits to hospital wards, carers’ homes, or community venues.
- Undertake follow up reviews with carers.
- Provide a range of preventative community-based support for carers
- Provide information & advice through the Cares Hub, through group activities, drop-in sessions and 1-1 support.
- Provide support and cover for the department manager.
- Provide robust, clear and accurate reports, being able to deliver and present findings to other parties.
- Promote and respect the diversity of carers and encourage cultural sensitivity
About You
- Experience of working with carers or in a health/social care setting.
- Knowledge of hospital discharge processes and the challenges carers face during this time.
- Understanding of grant funding or benefits systems is desirable.
- Strong communication and interpersonal skills, with the ability to work compassionately with people in challenging circumstances.
- Organised, self-motivated and capable of working both independently and as part of a team.
- Confident in using IT systems for case management, reporting, and communications.
- A good knowledge of the local area and services in Leicester.
- Friendly, personable, empathetic, professional, supportive whilst working with safeguarding guidelines.
- Driver with a UK driving licence having access to a vehicle and the ability to gain business insurance

- Working hours – 20 per week Monday, Tuesday 9am – 4pm & Wednesday 9am – 4.30pm
- Salary: £14,516.61 per annum - £13.92 per hour
- Based within Leicester City Centre – with a car parking space
- Fixed term contract 31 January 2026
