Support Worker

An exciting opportunity has arisen to join our Support Worker team, primarily working in our Dementia Day Centre at Plymstock, with the addition of cover at our Centre in Mount Gould and out in the Community. 

This role will involve working as part of a team, working closely with our management team, families, and friends of the people you are supporting alongside other health and social care professionals to ensure people are supported to lead the lives they choose.

Community Assessment Team Lead

The primary purpose of this post is to undertake assessments for new and existing customers for the purpose of planning and reviewing their home care needs in order to provide a package of holistic support.  This will include assessments undertaken as part of our commissioned hospital discharge packages.  You will be working closely with the hospital discharge team at Mount Gould Hospital and the Short Term Care Centre as well as internal staff.
