We have an exciting new opportunity to join our Discharge Team as a Rapid Responder.

Would you like to be part of a team helping to relieve the pressures on the NHS by supporting those leaving hospital?

This post involves providing a commissioned service as agreed with the NHS at the point of discharge for individuals living in their own homes. It will involve a variety of different task which may include, collection of patients from hospital, helping them to get to appointments, basic food preparation, light cleaning, shopping and collecting prescriptions for them. Successful individuals will be compassionate, organised and highly motivated to provide high levels of support to clients, ensuring that those initial weeks post discharge pave the way to returning to living independently at home.

Discharge Assessment Team Lead

We have an exciting new opportunity to join our Discharge Team as a Discharge Assessment Team Lead.

Do you have line management skills, coupled with a desire to assist older people in accessing services across the City of Plymouth?

The primary purpose of this post is to undertake assessments for patients who are referred to our Discharge Service on discharge from Hospital. The assessments are carried out to establish what holistic support is needed with a focus on reablement and preventing readmission. This is a commissioned service and as such you will work closely with the Discharge Teams at Derriford, Mount Gould Hospital, DAU and Short-Term Care Centre as well as internal staff.
