Key Worker Stockport: The Prevention Alliance
Salary £19,380 Full time, PRO RATA part time - Fixed term contract to May 2020.
There are two vacancies ranging: 15-37 hours per week to be worked flexibly to meet the requirements of the service
You will be directly employed by Age UK Stockport and will work in partnership with five other Alliance partners to deliver The Prevention Alliance (TPA) service within Stockport.
The Prevention Alliance mission is ‘Creating change together – through the strengths of people and communities’. We work with people to improve their independence and resilience through a person led approach, offering holistic advice, signposting, and dedicated workers to help people achieve positive change.
We enable people to explore and utilise their strengths to build their confidence. We encourage people to develop their own sustainable network of support, whilst sharing their skills and experience to give something back.
Enthusiastic and dedicated you will have a commitment to work as a positive team member within the Alliance. Ideally you will have the experience or the skills required to support a wide range of vulnerable people and households, be a good communicator and an ability to organise and prioritise your own workload is essential.

Support Worker (Tier 1)

To deliver high quality and supportive care to vulnerable people including older people to enable them to remain living in the Community.
Ensuring safe and appropriate support including personal care, administration of medication and moving and handling to clients and any other practical support deemed reasonable.
To provide general support to the client as part of flexible team including informal carers and liaising with other services as necessary.
Working at premises and carers homes located throughout the Borough of Stockport.

Working flexible hours by mutual agreement, at a salary of £8.20 per hour (out of hours rates paid.). Holiday pay will be based on hours worked.

Care Support Worker

To provide high quality person centred care and support to vulnerable people including older people to enable them to remain living in the community by providing care and support within the persons home and/or within the wider community. Covering tasks such as undertaking personal and internet shopping, support to attend appointments, social activities and other general practical support deemed reasonable.

Also to provide reliable relief to carers of older people through short periods of respite, to undertake those caring tasks normally undertaken by the carer during such periods of respite to provide general support to the carer.

Working at premises and carers homes located throughout the Borough of Stockport.

Working flexible hours by mutual agreement, at a salary of £7.50 per hour. Holiday pay will be based on hours worked.
