Sutton Health & Care

There is a number of services within Sutton Health and Care which you might be referred into: 

  • At Home Service 

A group of professionals with different skills who support people to get home and stay at home, rather than being admitted to hospital. 

  • Maximising Independence Service  

Previously Falls Prevention and the Older Peoples Assessment and Rehabilitation Team, they help for people to move safely in their own home. 

  • Community nursing 

A member of the community nursing team may visit you at home if you cannot get to the GP.  

  • Continence Service 

Supporting people to remain independent. They can also help with supplies of continence products. 

START (Short Term Assessment and Reablement Team) 

Up to six weeks of support from therapists and social workers can be available to help you maximise your independence and recovery.  

Community Teams 

  • Occupational Therapist (OT) 

Can recommend equipment or adaptations which help you be independent in essential daily tasks. 

  • Physiotherapist (PT) 

Can recommend equipment and exercises to help you move safely. 

  • Speech and Language Therapist 

Can give advice on how to swallow safely or help if you are having difficulties with speaking. 

  • Community Dieticians  

Based within Pharmacies or in GP surgeries, can help you with dietary advice to consider what you are doing that is good for your health and help you to identify areas you may like to work on to protect or improve your health.  

Sutton Community Virtual Ward  

Brings together clinicians, community partners and other relevant health professionals in three weekly meetings to discuss and coordinate a support package for patients who are considered vulnerable and at risk of hospital (re)admission. 

Social Worker / Adult Social Care 

May organise your home care and can advise you on your funding options for this. Can also help you to access other support provided by the London Borough of Sutton. 

Home Care Agency  

The agency who may visit you to help with essential daily tasks. They make up the ‘Care Package’. 

Age UK Sutton Home from Hospital 

Short term practical and emotional support to help you in your recovery, and support to make longer term plans if needed. 

While you are still in hospital, if you are unsure of who these people or services are - and their role in your care - please ask your hospital Discharge Coordinator.

Hospital discharge feedback and complaints

Healthcare staff and Hospitals are always very keen to get feedback on the standard of care that you have received.   

If you are unhappy or worried about any aspect of how your hospital discharge has been handled, firstly speak to the staff involved to see if the problem can be resolved informally.  

The PALS team at the hospital can provide confidential advice, support and information on health-related matters.

If you wish to raise a formal complaint about your hospital discharge, follow the NHS complaints procedure.
