Home from Hospital Coordinator

Hybrid role with visits to client homes and other locations.

£11.80 per hour

Special Conditions:

Local travel during work may be required, for which mileage allowance or public transport costs will be paid. 

Must have access to own transport

 

Occasional evening and weekend work may be necessary, for which time off in lieu on an hour-by-hour basis is allowed.

 

The post is subject to a satisfactory Enhanced Disclosure and Barring Scheme check for Adult Workforce.

 

A mobile phone and laptop is supplied.

JOB SUMMARY

 

To deliver the Home from Hospital Service - ensuring that older people or people with a physical or sensory disability have practical and emotional support for a 3-6 week period following discharge from hospital to reduce their likelihood of readmission by improving their overall health and wellbeing.

Main Duties:

 

  1. To deliver the Home from Hospital scheme, ensuring that clients are supported for 3-6 weeks following discharge from hospital, delivery will mainly be face to face in their own homes.
  2. To highlight any clients requiring more support and agree a support plan post the 6 week period with the Home from Hospital supervisor.
  3. To receive referrals from hospital contacts and ensure that discharges are supported within one working day of notification.
  4. To carry out risk assessments and set up person centered support plans that enable individuals to live in their own homes following discharge from hospital
  5. To enable individuals to maintain independence by assisting them to continue with everyday tasks/routine at home (shopping, prescription collection/arranging, paying bills, GP appointments and light household tasks).
  6. To provide emotional support, information and signposting of clients to other organisations, either face to face or by phone.
  7. To ensure clients health and wellbeing is a priority and safeguarding policies and procedures are always followed.
  8. To liaise with all referrers and highlight the service to health and social care teams and other interested parties in raising the profile of the scheme.
  9. To provide written records of each visit, maintain confidentiality of the client and update customer database with all relevant information.
  10. To ensure that the all client feedback is proactively gathered, evaluated and influences future service delivery.
  11. To undertake relevant training to maintain skills and knowledge necessary for this post.
  12. To comply with all Age UK Berkshire policies and procedures and with all relevant legislation. To comply with all provisions of the Age UK Berkshire Information Governance policy and procedures.
  13. To carry out any other duties, commensurate with the rate of pay and grading, as may from time to time be assigned by the Trustees of Age UK Berkshire.
