Salary: £24,720 FTE

The Service:
The Integrated Care programme (ICP) is a free service designed to support older people aged 50+, and their carers', in East Lancashire.
The service is aimed at coordinating support from a wide range of voluntary, health and social care service providers for those living with long-term health conditions and complex needs.
The Job:
As a Personal Independence Coordinator, you’ll be supporting people in their own homes, providing a continuous and supportive presence for up to 12 weeks, helping individuals to identify what they need and create a personalised plan enabling them to self -manage their long- term health conditions and enhance their independence. You will help to ensure that services and support are delivered in a personalised, coordinated, relevant and timely way for the individual.
Bringing together voluntary organsiations and Health & Social care services in East Lancashire, this is a role that really makes a difference to people’s lives, health, and wellbeing by providing a combination of clinical and non-medical support for older people who are living with multiple long-term health conditions, and who are at risk of recurring unplanned hospital admissions.
The Personal Independence Coordinator forms a key role within a multi -disciplinary team and The Integrated Neighbourhood Team, providing regular contact, liaison and support planning to identify patients, their carers and family.
About you:
- You will have knowledge or experience of Health, Social Care or Wellbeing.
- You will have a customer focussed approach with a caring & empathetic attitude.
- You will have confidence to collaborate with other Health & Social care professionals, including GP’s
- You will have excellent communication skills, both verbal and written, with an ability to build relationships internally and externally
- You will be able to demonstrate effective planning and problem-solving skills
- You will have an ability to work alone and, in a team,
- You will be able to maintain professional boundaries
Full training will be provided.

- The nature of this post will require travel across the area. A Full Driving Licence, appropriate Insurance cover and use of a suitable car are essential. Travel expenses will be paid for all work-related journeys.
- DBS: This post is eligible for an enhanced DBS check at time of appointment; all the associated costs will be met by the employee.
- This role is not eligible for UK Visa Sponsorship - the successful applicant will need to have a pre-existing Right to Work in the UK in order to be offered an employment contract.
