Age UK Shropshire Telford & Wrekin have joined forces with Telford and Wrekin Council for Voluntary Service (CVS) and Taking Part to provide a holistic service for people with social care and health needs in Telford.

The three voluntary sector organisations have formed a partnership to provide information, advice and guidance to adults and carers of all ages, who need help with health or social care.

Debbie Gibbon, Chief Executive of Telford and Wrekin CVS, said: ‘This is a great opportunity for us to provide an integrated approach to help adults and all aged family carers with social care needs get the right help at the right time. We all bring different areas of expertise to the partnership.’

‘As voluntary and community sector organisations we have always worked closely so we are delighted to have secured the contract formalising the partnership under one umbrella to better support people living within Telford and Wrekin. The new service, which is called the Telford Wellbeing Independence Partnership, will be working across all sectors to ensure people live well closer to where they live.’

Telford & Wrekin Council, who fund the project with Telford and Wrekin CCG, said: ‘This service demonstrates a new integrated and seamless service delivery model which means that the person will need to ‘tell their story only once’ and have a robust first point of contact with appropriate swift response for clients regardless of need, disability, role and age. The service aims to simplify access to information, advice and guidance as well as promoting independence.’

The new partnership starts on 1 October 2019: for more information contact Telford and Wrekin CVS on 01952 262 066.
