Starting this May! Bringing Memory Lane - our Dementia and Memory Support Service - to the comfort of your own home.
Memory Lane at Home provides a dual-benefit companionship and outreach service.
We focus on enhancing the well-being of individuals living with a dementia diagnosis or memory difficulties, while simultaneously providing family carers with a reliable, much-needed break from their caring duties.
Community Engagement & Outreach
We believe in maintaining “community continuity.” Our support workers help clients stay connected to the world around them by:
• Accompanied Outings: Visits to local cafes, shops, garden centres, libraries, parks, or other places of interest.
• Maintaining Hobbies: Providing the support required to keep attending clubs, choirs, faith groups, or other social activities.
• Social Confidence: Helping individuals navigate public spaces safely and with dignity to prevent social isolation.
In-Home Companionship
When a client prefers to stay at home, we provide person-centred engagement:
• Cognitive Stimulation: creative arts, reading, puzzles & memory games.
• Reminiscence Work: Sharing life stories and looking through photo albums.
• Practical Support: Preparation of light snacks and drinks.
• Reassuring Presence: Providing a period of “quiet companionship” - allowing the person to rest or go about their day as they wish, safe in the knowledge that someone is at hand if needed.
Registration and Assessment
To ensure the highest standards of safety and care, all customers must be registered with the service before any visits can be booked.
• Home Assessment: A Service Coordinator will visit you to complete a thorough assessment. This covers medical history, personal interests, routines, and any specific safety requirements for outings.
• Matching: Once the assessment is complete, we identify a visiting support worker whose personality and interests align with the client.
• Booking: Following registration, you may begin booking sessions.
Frequently Asked Questions (FAQs)
Do I need to commit to regular visits?
Most of our clients book regular weekly visits (the same day and time each week). This continuity is highly beneficial as it helps develop a stable routine and a strong rapport with the visiting support worker. It also ensures the carer has a reliable and scheduled time to rest or run their own errands.
Is there a minimum visit duration?
Yes. The minimum amount of time that can be booked for any single session is two hours.
Can I book a one-off visit?
Yes. While we recommend a regular schedule, once the initial registration and assessment are complete, you can request ad-hoc “one-off” visits for specific appointments or family events (subject to staff availability).
Do you provide personal care?
No. Memory Lane at Home is a social companionship service. Our staff do not provide clinical or personal care, such as assistance with medication, bathing, or toileting.
Costs and Fees
• Registration Fee: There is a one-off fee to cover the initial home assessment.
• Hourly Rate: Visits are charged at a standard hourly rate (please contact us for current pricing).
• Mileage: If the support worker uses their vehicle to take a client out into the community, mileage is charged at 45p per mile. This is added to your invoice and is paid in full back to the support worker to cover their travel costs.
• Outing Expenses: If the support worker is taking the client to an event with admission fees or to a cafe/restaurant, the client must have the means to meet these costs at the time. Please note that the client is also responsible for covering any admission costs for the support worker too.
