Home from Hospital: Community discharge support volunteer

Volunteer role description 

As a Home from Hospital volunteer, you will support older people 65+ in Bolton post-hospital discharge and older people 50+ to avoid an admission to hospital with practical, emotional, and welfare support, enabling safe, confident recovery at home.

Key responsibilities 

• Visit clients in their homes, following an agreed support plan (typically up to 6 weeks).
• Provide welfare support, noting safety or well-being concerns.
• Help with:
  • Home safety and warmth checks
  • Prescription collections
  • Shopping for essentials 
  • Light housework e.g. tidying, dishes, putting away shopping
  • Sorting post or bills
  • Encouragement with physio exercises
  • Attending to book medical appointments
  • Delivering our Take Home and Settle and/or Medication Delivery services
• Report any concerns promptly to the Service Coordinator or Team Leader.

Please note this role does not involve any personal care.

What we're looking for

We're looking for someone who is... 

• Comfortable working independently in clients’ homes
• Empathetic, patient, and compassionate
• Good with basic IT skills, including data entry
• Willing to learn to use the CRM system (training provided)
• Able to work well independently and as part of a team
• Aware of safeguarding and welfare, willing to report issues
• Interested in (or has a background in) health and social care or community work
• Respectful of confidentiality and has a non-judgmental attitude
• Reliable, punctual, and committed to the values of Age UK Bolton
• Commited to training and a DBS check required
• A car driver with access to a vehicle (preferred)

Training and support

• Full induction and role-specific training provided
• Ongoing supervision and support from the Home from Hospital Team
