Community Memory Support Leader

19 hours per week @ £11.30 per hour.
One year fixed-term contract with possible extension
Current driving licence and own car needed

Enthusiastic, well‑organised person needed to provide personalised emotional support, guidance and information to older people (in their own homes) affected by memory loss/dementia, assisting them to identify their needs and access other services, enabling them to maintain their independence, choice and control.  You will be a confident communicator, have excellent interpersonal skills and good knowledge of Microsoft Office.  Experience of working with people affected by memory loss is essential.  Current full driving licence and own car needed for the role.

This role is part of a new countywide-based project to improve mental, physical well-being and socialisation for people with mild to moderate memory loss/dementia through the dissemination of healthy living and healthy eating, information and advice, and encouragement to participate in activities which promote physical and mental well-being and prevent social isolation.
