Pathway 0 Team Leader
The Better at Home Team Leader will contribute to the reduction of hospital readmissions by ensuring the personalised care offer from social prescribing is fully utilised, by building on the Pathway 0 pilot:-
This role will be focused on supporting the Pathway 0 Project, and occasion support on other project pilots.
The Pathway Zero pilot in Herts funded through Hertfordshire County Council to March 24 via HertsHelp) whereby all patients over the age of 65 receive a call following their discharge from Hospital within 24 hours, with a follow up call at 5 days as appropriate.
Job Title: Pathway 0 Team Leader
Location: Home Based - Hertfordshire - supporting Watford General Hospital, Lister Hospital, Princess Alexander. Also the option to work in the Hemel office if desired.
Hours: 35 hours per week
Line Manager: Better at Home Manager
Responsibilities:
- Support with contacting patients on the discharge Pathway 0 list.
- Patients are kept mobile and active as far as possible and are offered rehabilitation (exercise, weight loss advice, health services, clubs, etc to improve outcomes, as appropriate).
- Their health is not compromised by inability to afford heating or access food etc.
- Ensure that the staff and volunteers contain a good balance of knowledge and skills.
- Create a learning environment, ensuring that knowledge and skills transfer easily throughout the project.
- To ensure people being discharged from acute trusts have a personalised offer of practical help or wellbeing support, to prevent readmissions.
- Their morale and wellbeing including financial wellbeing is supported.
- Developing consistent and effective referral processes.
- Ensuring staff & volunteers are made more aware of the VCFSE (Voluntary, Community, Faith and Social enterprise).
- Contribute to the development of targets.

Salary: £22,500 per annum

Better at Home Call Handler
The Better at Home Team has been set up to support the Acute Trusts manage hospital discharges as well as waiting lists via a wellbeing telephone call. The two projects are Waiting Well and Pathway.
Waiting Well’, supports a cohort of patients identified by the trust who are waiting on an operation and the second is ‘Discharge Support’, focusing on a) telephone follow up to 65+s discharged on pathway zero and b) 2 weeks’ wraparound care for patients on other pathways who need non-clinical help to build resilience once home, again utilising the support available in the community to reduce readmissions.

Location: Hybrid home/office based
Hours: 7 hours per week (to be discussed)
Line Manager: Better at Home Team Leader
Responsibilities:
- Contacting via telephone, discharged patients, from the following hospitals: Watford General, St Albans, Hemel Hempstead, Princess Alexandra & Lister, who are 65 years of age and over.
- Contact via telephone a list of patients who have been identified as being on a long waiting list for a procedure.
- Completing spreadsheet patient information, on Microsoft Excel, via SharePoint.
- To identify what support the discharged patient may need, to help them settle back into their home safely and securely.
- Referring the patient into Herts Help for further support and services to be set up, such as Meals-on-wheels, mobility aids, shopping & medication collection or delivery, cleaning & laundry, key safes or handy man jobs, befriending or bereavement support, assistances with paperwork or technology, etc.
- Complete general admin duties, such as manage emails, case studies and Staff report.
- Be proficient in using Microsoft packages: Outlook, Teams, Word, Excel, SharePoint.
- Complete case studies, 2 per quarter.
- Daily contact & communication with colleagues online via Microsoft Teams.
- To liaise, when necessary, with Community Link Workers, Personal Assistants and Floating Support Workers
- To adhere to all regulatory and statutory obligations and AUKD’s policies, procedures, and guidelines.
- To actively market AUKD, and promote a positive, personal, and professional profile, ensuring the good reputation of AUKD always.
- Complete other hospital call lists, as and when needed by HCNS/Herts Help.
- When making calls, being able to recognise the following:
- Protect the confidentiality of all information relating to the client and not divulge information to anyone who is not authorised to receive it.
- To promptly report to the office of any issues concerning the care, support, well-being or behaviour of the client and update records accordingly.
- To recognise the signs of abuse and immediately report abuse or suspected abuse to your line manager or if not available a Senior Manager.
- To report any complaints to the office.
- To seek out best practice and look at innovative ways to improve the quality and efficiency of service delivery.
- Attend and participate in occasional team meetings and any other relevant meetings.
- To attend in house and external training pertinent to the role.
- To aim to ensure everyone has equal treatment and equal access to services.
- Any other duties requested by your line manager which are within the scope of the post.

Young at Heart Club Assistant
Our Young at Heart Clubs and Memory Loss Clubs provide a safe, caring, stimulating environment for older people in the local community. This role will provide cover for the Club Leaders during periods of annual leave and sickness. It will operate within agreed standards set out in the partnership agreements between Age UK Dacorum, Age UK Hertfordshire and Hertfordshire Community Foundation.

Hours: 5 - 10 hours per week
Line Manager: Clubs Manager
Responsibilities:
- To support the Club Leader with the planning, organising and delivering the activities for the day. These activities should be stimulating, inclusive and engaging. They should include: physical activity, creative activity, cognitive games, health improvement and awareness sessions and outings;
- To assist in setting up the centre for the day, and cleaning and tidying up at the end of the day, which may involve moving furniture;
- Support clients with mobility by offering an aid, a shoulder or an arm to steady on for support with balance;
- To record accidents, unusual behaviour or incidents and report to the Club Leader;
- Maintain strict client confidentiality at all times and adhere to GDPR;
- Clients attending the clubs will be self-managing in toileting and mealtimes, however, there may be rare occasions where minimal assistance is required;
- To follow AUKD policy and processes at all times, including Health and Safety;
- To attend team meetings, supervision and training sessions as required;
- To provide transport for clients if and when required, this will mean that you must hold car insurance for business use;
- To escort clients to the dining area and help to distribute midday meals and refreshments;
- To undertake any other reasonable duty as required by the Club Leader or the Clubs Manager;
Job Type: Part-time
