Personal Independence Coordinator

Base:             Nelson

Salary:           Full Time Equivalent £24,000

Hours:           14hrs over 2days or 15hrs over 3days

Term:             Fixed until 31/03/25

Age UK Lancashire is looking for a Part time Personal Independence Coordinator, to cover Pendle West in Nelson and surrounding district. This is a rewarding and satisfying role where you can make a hugh difference to people’s lives. You will be delivering a coaching and mentoring approach and offer emotional support to undertake an assisted self-assessment called a Guided Conversation, providing a supportive and empathic presence to people with complex needs and long-term health conditions, setting personalised goals with the individual to ensure that the service is delivered in a coordinated, relevant and timely way.

This forms a support plan that provides a wrap- around service to the client. This is not a personal care role.

It’s a varied role where you will be liaising with the Primary care network of professionals, including GP’s delivering client updates to the Integrated Neighbourhood Team (INT ) with the support you have put in place reporting to the service manager.

Open to keeping your skills and knowledge up to date and we will ask you for your opinions and create opportunities for you to get involved in shaping what we do and how we do it.

The nature of the post will require travel across the area. A full driving licence, appropriate insurance cover and use of a car are essential.

DBS: This post is eligible for an enhanced DBS check and all the associated costs will be met by the employee.
