We are working with Northamptonshire Carers and Serve, to provide support to people aged 18 - 65 years in the Wellingborough and District Primary Care Network.

Our seven Social Prescribing Link Workers work with over twenty GP surgeries in the north of the county.

Our Social Prescribing Link Workers are based in GP surgeries. 

  • We visit people at home or go for a walk with them in their local area to find out how we can help
  • We work to empower families so that they can support their relative
  • We encourage our clients to join local groups and activities they they will enjoy
  • We will refer people to other Age UK Northamptonshire services where appropriate

An example of social prescribing in practice

Denise*, a patient at the GP surgery, was referred to our Social Link Worker as her life was being limited by the pain she experienced and she was feeling socially isolated.  Our link Worker Chris spent time getting to know Denise, to find out what she would like to achieve and what support she might need to achieve it. 

Chris works in partnership with Rockingham Forest Trust to organise regular walks at Stanwick Lakes for people who are struggling with social isolation and mental health challenges.  Chris encouraged Denise to take part in an activity which would be good for her wellbeing and Denise now joins others at Stanwick Lakes for a walk and coffee every week.  Denise benefits from the social interaction.  When she isn't able to take part in the walk she still comes along to the session when she can, using the time for some mindful meditation in the peaceful gardens.

Denise has also attended an art class that Chris encouraged her to join.  Chris also arranged for Denise to have some help with household chores, which enables  her to focus her time and energy on socialising more.  Denise is pleased to have achieved her goal of getting out more and expanding her social circle.  Denise now hopes to try some volunteering opportunities at Rockingham Forest Trust.
