Kettering General Hospital - KGH Support Service, Age UK Northamptonshire provides practical and emotional support to patients age over 55 years in the A&E Department, Dementia Wards and Discharge Lounge, working in collaboration with other health care professionals. We also support professionals in the hospital by attending to patients' non-medical needs.
Nene Clinical Commissioning Group - Age UK Northamptonshire is funded to provide a Hospital Aftercare Service, Carers Sitting Service and Collaborative Care Teams which support older people after a stay in hospital and help to prevent unnecessary re-admissions.
