Living Well Support Service Wellbeing Worker-East Lancs
Location: Royal Blackburn Hospital
Salary: £24,661 FTE
Hours: 18hrs per week. (Working between the hours of 9-7pm on a 6hr shift X three days per
week on a rota basis over 7 days per week including 1 weekend in 3, along with some bank holidays)

If so, we are looking to recruit a Wellbeing Worker to join our existing Living Well Support Service team, working across East Lancashire.
Age UK Lancashire’s Living Well Support Service provides invaluable support to clients over the age of 18, helping them attain or regain independence after a hospital stay or prolonged illness. Our service is integral to support hospital discharges, preventing unnecessary hospital admissions, and may reduce the need for residential care. In the last 12 months, the service has supported over 9,000 people across the County.
As a Wellbeing Worker you will:
- Take patients home directly from the hospital and settle them back at home, ensuring they have everything they need to remain at home.
- Provide continued support for up to 6 weeks, to help them retain or regain their independence.
- You will use a solution focused approach to ensure that people can access the help that they need when they need it, either from Age UK Lancashire or from other community-based support.
To be successful in this role you will:
- Have a caring and empathetic attitude with a focus on helping people to maintain independence
- Be able to provide a person-centred, flexible and responsive service
- Have experience of managing your own workload, using decision making skills to manage and prioritise
- Be able to work well within a team and able to use your own initiative
- Have excellent communication skills with an ability to build relationships with individuals using the service, and other health care professionals and colleagues.
- Be able to use your digital skills to enable you to manage your time efficiently and effectively, including Microsoft applications such as Outlook and Excel and everyday use of iPad and smartphone devices.
- Able to manage a busy workload, keeping accurate records and conducting assessments
- Able to undertake general physical activity in people’s homes (often over 2 floors) for example providing light domestic duties
- A full driving Licence, with appropriate insurance cover, and be willing to transport patients in your own car.
