This initiative focuses on early intervention and hospital avoidance, helping people at risk of poor health to live independently and access support before reaching crisis point.
What’s new?
- Launch of the Avoidance Pathway – supporting individuals not currently in hospital
- Aims to prevent unnecessary admissions and frequent Urgent Care or A&E visits
- Working with Primary Care Networks and Integrated Care Communities (ICCs) to identify people earlier through community-based referrals
- Expanded team in Lancaster and recruitment of Volunteer Health Champions
What can service users expect?
