Age UK Lancashire’s Living Well Support Service is designed to help older people return home safely after a hospital stay, prevent unnecessary hospital admissions, and provide urgent support following a significant life event. Our goal is to promote independence, improve wellbeing, and ensure you have the right support to live safely and comfortably at home.

We offer two levels of personalised support:

Tier 1 – Time limited support of up to 3 hours, an assessment of the individual in the home environment to ascertain ongoing support needs and may include transport home from hospital.

Tier 2 – A Follow up support package providing essential information, advice, and practical assistance with a range of aspects including but not limited to: home safety, access to aids for daily living, falls prevention info, medication support, fire safety checks, finance issues, benefits advice, and carers support.
