Location:  Waterside House, Irthlingborough and community based

Rate of pay:  £16.55 per hour Lead two days; £15.55 per hour SPLW two days

Hours of work:  30 hours per week

SPRING is a partnership of social sector organisations, commissioned by Northamptonshire Health & Care Partnership, to improve the wellbeing of people in Northamptonshire using Social Prescribing.

About the role

The key purpose of this role as a Lead is to effectively manage the day-to-day activity of Age UK Northamptonshire’s SPRING Social Prescribing Service to ensure a high quality, appropriate, and relevant service is delivered.

Social prescribing empowers people to take control of their health and wellbeing through referral to SPRING Link Workers who take a holistic approach to an individual’s health and well-being.  SPRING connects people to diverse community groups and statutory services for practical and emotional support.  The SPRING Social Prescribing Link Worker will develop and promote activities for clients of SPRING services.

You will line manage the established team of four Social Prescribing Link Workers across the north of the county and one Administrator, and also contribute to the recruitment, management and training of volunteers.  We will expect you to forge strong links with community organisations to build a comprehensive database of local resources and keep this information accurately updated, working collaboratively with local diverse partners.  Keeping accurate, up to date records of caseload and adhering to confidentiality and information sharing protocols are important aspects of this role.

You will be expected to promote positive relationships with our partners and stakeholders: adult social care, hospitals, GPs and other organisations that support older people in the county.

About you

We are looking for someone with experience of managing and working in a team and also able to work proactively on their own initiative.  We are looking for someone with excellent verbal communication skills and  the ability to liaise with other professionals.
