Social Prescribing Winners!

We are delighted to say that our INTERACT and Norwich and Broadland Social Prescribing projects were both proud recipients of awards, at the recent Social Prescribing Conference on 1st December.

INTERACT won ‘Social Prescribing Wider Determinants Collaboration of the Year’

“You truly demonstrate collaboration and partnership working which addresses the wider determinants of health to enable real change for our communities.   This award allows us to acknowledge your achievements, thank you!”

INTERACT is a multidisciplinary project involving Norwich City Council, ourselves, Norfolk Citizens Advice, Voluntary Norfolk, One Norwich Practices and an Integrated Care Coordinator from Norfolk County Council.  

 

Norwich and Broadland Social Prescribing team won two awards:

 ‘Social Prescribing Team of the Year’ – Living Well Team

“You pride yourselves on holistic approaches to care, which focus on outcomes and staff wellbeing. You face challenges head on, drive inclusivity, offering excellent patient centred care built on strong engagement and relationships with your communities and ICS partners. This award allows us to acknowledge your achievements, thank you.”

and

‘Social Prescribing Innovation of the Year’ – Living Well Team

“You demonstrate leadership by serving as a role-model and mentor to your peers. Showing initiative and promoting positive change in social prescribing delivery. You actively advocate for the care of patients and their families and contribute to improved patient outcomes through commitment to delivering high-quality care. This award allows us to acknowledge your achievements.”

 

The Norwich and Broadland Social Prescribing Project is a collaboration between Norfolk Citizens Advice (Lead Partner), Age UK Norwich, Shelter, Mancroft Advice Project and Equal Lives.

Jo Willingham (Age UK Norwich Information, Advice & Welfare Manager), Vanessa Knell (Age UK Norwich/INTERACT social prescriber), Lee Brook (Norfolk Citizens Advice Social Prescribing Project Lead), Janette Woodcock (Norfolk Citizens Advice Living Well Team Leader) and Joel Watkins (Norfolk Citizens Advice Living Well Worker) who was also one of the Social Prescribers of the year.
