About The Role

Are you ready for a fulfilling role that makes a real difference in the lives of older individuals during their times of need? We have an exciting opportunity for a Crisis Carer to join our remarkable Crisis Team.

Our Crisis Team operates seven days a week, covering a span from 8:00 am to 8:00 pm. Shifts are available in the morning (8:15 am to 12:45 pm) and in the afternoon (3:45 pm to 7:45 pm). We specialise in delivering short-term, person-centered care to people in later stages of life, whether they are returning home from the hospital or facing unexpected challenges. As a Crisis Carer, you will provide essential services within the comfort of the client's home, including shopping for necessities, light meal preparation, collecting prescriptions, aiding with medication, and offering support with personal care.

This role is immensely gratifying for individuals who are compassionate and dedicated. You will become an integral part of our Crisis Service team, ensuring the safe and efficient delivery of care within our clients' homes.

Key Duties and Responsibilities:

  • Referral Management: Receive direct referrals from the Coordinator on duty
  • Client Support: Offer in-home support to clients within the scope of our services, including providing low-level personal care as required.
  • Check Phone Calls: Conduct scheduled phone check-ins as agreed upon with the Manager to maintain ongoing communication and client well-being.
  • Risk Assessment: Perform basic risk assessments as part of the 1st Call visit pack, ensuring that potential risks are identified and addressed promptly.
  • Communication: Immediately report any issues or concerns. Maintain open and effective communication with the Coordinator regarding ongoing cases.
  • Team Collaboration: Collaborate with other Care Assistants.
  • Record Keeping: Maintain clear and accurate records of all client interactions and share these details with the Coordinator on duty at the end of each workday.
  • Documentation: Ensure the thorough completion of all required forms and documentation.
  • Professional Development: Attend team meetings and actively participate in training and other development activities as appropriate to enhance your skills and knowledge.
  • A valid driver's license and access to a car are required for this role.
