Community Link Worker – Arundel/Bognor
Hours: 24 hours a week with occasional evenings and weekends
Salary: £15,366 pa (£23,690 FTE)

About The Role
We have an exciting opportunity within our newly established Community Link Service. As a Community Link Worker, you will be working closely on a 1:2:1 basis with individuals to facilitate their access to local community services. Whether it's assisting with benefit applications, providing housing advice, or combating loneliness by encouraging social engagement. Your efforts will contribute to improving the quality of life for many who lack local family support.
As a valued member of the multidisciplinary NHS team within Sussex Community Foundation Trust, your work will directly impact the well-being of community members, helping them remain in their homes and preventing unnecessary hospitalisations. This service supports individuals aged 18 and above, ensuring that adults of all ages receive the necessary support to thrive. This is an extremely rewarding role making a big difference to local people, keeping them out of hospital and in their homes.
What you need to be successful in this role:
- You need to have strong communication and interpersonal skills, talking and engaging with clients, colleagues, and professionals.
- You’ll need to be good at helping to find solutions for clients.
- You should know how to refer people to local services that will help people further (this will be part of your training).
- Experience of working in health care is helpful but not essential.
- You need to know to have good IT skills to be able to keep client records update to date as well as good written skills.
- You need a driver's licence, a car, and be okay with traveling (we'll pay for the mileage outside of you base).
- You should be comfortable working in people's homes, the community, hospitals or offices.
- You’ll need to be flexible to meet the needs of individuals and the multidisciplinary team, with a positive and can-do attitude.
- Ability to work some weekends as part of a rota.
- You should be good at managing your work and competing demands, sometimes it’s very busy.
- Essentially, you should want to help local people be healthier and happier!
