Hours: 37 hours a week (1 year FTC Maternity cover)
Salary: £26,775
Location: Bognor/Chichester/Hybrid

Join Our Team as a Community Support Worker!
We’re looking for a dedicated and empathetic individual to join our Prevention Assessment Team (PAT) as a Community Support Worker on a 12-month maternity cover contract. This vital role is part of a multi-disciplinary team led by West Sussex County Council (WSCC) and works closely with local partners to support residents who need guidance but don’t meet the thresholds for statutory services.
As a Community Support Worker, you’ll be providing crucial information, advice, and support to individuals with identifiable needs, helping them maintain their independence and promoting their health and wellbeing. You’ll play a key role in preventing crises by connecting people with the right services at the right time.
What You’ll Be Doing:
- Promoting Health & Independence: Holding wellbeing conversations and supporting people to help themselves using a person-centred approach.
- Signposting & Support: Helping individuals access health, social care, and community services – and completing the necessary assessments and paperwork.
- Coordinating Care: Working collaboratively with the voluntary and community sector through our Community Led Support model.
- Preventing Escalation: Acting as a point of contact for those at risk and liaising with other professionals to prevent crises and deterioration.
