Salary: £16,282 (£26,775 FTE)
Location: Hybrid with a base at Brighton - Preston Park Community PCN

About the Role:
As a Social Prescriber, you will work within our Primary Care Network community team or GP practice, collaborating with health professionals and voluntary sector partners to provide personalised support for patients aged 18 and over. This role isn’t just for older adults, it’s for anyone who could benefit from a little extra help to stay connected, independent, and engaged in their community.
Key Responsibilities:
- Work with patients to understand their needs and develop personalised solutions.
- Connect people to local services and activities that support health and wellbeing.
- Collaborate with GPs, voluntary sector partners, and other teams to find creative ways to keep people independent and socially connected.
- Support patients to manage their health and care needs, encouraging self-care and confidence.
- Keep accurate records and gather data to demonstrate the impact of our service.
- Work alongside a team of enthusiastic volunteers to extend our reach and capacity.

We’re looking for someone who is compassionate, proactive, and a great communicator. You’ll need to be organised, understand the importance of personalised care, and be committed to helping people make positive changes in their lives.
